Please include:

1. Name, affiliation, name of advisor, advisor's e-mail address.

2. Thesis topic and SHINE topic(s) of interest. (See www.shinegroup.org for session information)

3. Level of funding required:
   a. Registration    b. meals    c. hotel (shared room)    d. airfare

4. Brief paragraph of support from advisor.

Please note that funds are limited and that requests for funding are due 1 week prior to the registration deadline (i.e. May 5).
